The CST exam, or Certified Surgical Technologist exam, is a national certification examination designed to assess the knowledge and skills required for entry-level practice as a surgical technologist. Certification validates that an individual has completed the necessary training and possesses the core body of knowledge crucial for patient safety and professional competence in the operating room.
The CST exam is administered by the National Board of Surgical Technology and Surgical Assisting (NBSTSA). This organization is solely responsible for developing the content outline, maintaining eligibility standards, and issuing the CST credential.
The CST exam contains 175 total multiple-choice questions. This breaks down into 150 scored questions that determine your result and 25 unscored "pretest" questions that are used by the NBSTSA for statistical analysis and future exam development.
The 150 scored questions are distributed across three main domains of surgical practice:
The full content outline is provided by the NBSTSA.
Perioperative Care carries the highest weight, accounting for approximately 97 questions (or over 60%) of the scored examination. This domain covers all aspects of care, including Pre-operative Preparation, Intraoperative Procedures, and Post-operative Procedures.
The Basic Science domain, which includes about 30 questions, tests foundational knowledge in Anatomy and Physiology, Microbiology (especially surgical infection control), and Surgical Pharmacology (e.g., drug classifications and dosage calculations).
The Ancillary Duties domain covers essential, non-procedural knowledge. Topics include Administrative and Personnel responsibilities (e.g., ethics, communication, documentation) and Equipment Sterilization and Maintenance (e.g., troubleshooting equipment, decontamination, and sterile processing).
Candidates must be a graduate of a surgical technology program that is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES), or have completed an equivalent military surgical technology program.
You must first create an account and apply on the NBSTSA website and pay the fee. Once your eligibility is confirmed (allow 2-4 weeks), the NBSTSA sends an Authorization to Test (ATT) letter, allowing you to schedule your appointment with the testing vendor (PSI) at a secure testing center.
Preparation should be comprehensive. Use the official NBSTSA Examination Content Outline as your blueprint, focus on the high-weight Perioperative Care domain, utilize recommended reference textbooks, and take Prephow CST practice tests to simulate the 4-hour test environment.
Most successful candidates find that a structured study plan lasting 3 to 6 months is necessary to effectively review all content areas, practice questions, and allow time to focus on weaker subjects identified through practice tests.
The CST certification must be renewed every two years (24 months) to remain current. This two-year cycle was implemented to align with industry standards and maintain competency.
