The COTA (Certified Occupational Therapy Assistant) exam is administered by the National Board for Certification in Occupational Therapy (NBCOT) for individuals who want to become certified occupational therapy assistants in the United States. Passing this exam is required by many employers and can lead to increased job opportunities, and ten states currently accept NBCOT certification for fulfilling state licensure requirements.
To be eligible to take the COTA exam, you must have successfully completed the academic and fieldwork requirements of a two-year Occupational Therapy Assistant (OTA) program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE).
The exam is taken on a computer and consists of 200 multiple-choice questions. However, only 170 of these are scored; the remaining 30 are unscored pretest questions. The test is presented in three timed sections, and you have 4 hours to complete it.
You must apply directly to NBCOT through their MyNBCOT online system. Once your application is approved and your academic institution confirms your graduation, NBCOT will issue you an Authorization to Test (ATT) letter. You then use the information in the ATT letter to schedule your exam appointment with the testing vendor, Pearson VUE.
The exam covers three main areas:
More than half of the exam focuses on selecting and implementing interventions, so that's where you should spend the most study time.
Successful candidates typically use a combination of:
Yes, If your education was completed in a country outside of the U.S. and your programme is not automatically recognised as meeting domestic accreditation standards, you may need to go through a process like the OTED (Occupational Therapy Educational / Credentialing Review) to verify substantial equivalency before being eligible to apply.

Prephow’s Occupational Therapy Assistant practice test honestly wasn’t worth it for me. It didn’t feel very close to the real NBCOT COTA exam or cover key areas like activity analysis, therapeutic exercise, and mental health in a helpful way. For the price, there are better OTA prep resources that match the actual exam content and format more closely.
