The ABIM (American Board of Internal Medicine) Exam is a certification test that assesses the knowledge, diagnostic reasoning, and clinical judgment of physicians who have completed training in internal medicine or its subspecialties. Passing the ABIM exam demonstrates that a physician meets nationally recognized standards of excellence in internal medicine.
Eligibility for the ABIM Certification Exam requires completion of a residency program in internal medicine accredited by the ACGME (Accreditation Council for Graduate Medical Education) and possession of a valid medical license. Applicants must also be in good standing with their training program.

There are up to 240 questions in total, spread across four blocks of up to 60 questions each. Questions are scenario-based and aligned with ABIM objectives, covering a mix of multiple-choice and performance-style prompts.
Candidates can register for the ABIM Exam through the official ABIM website by creating a physician portal account. Registration typically opens several months before the testing window, and early registration is recommended to secure a preferred exam date and location.
Effective preparation typically includes reviewing core internal medicine content, taking Prephow ABIM practice questions, and using ABIM-recommended study materials. Most physicians study for 3-6 months using board review courses, textbooks, question banks, and practice exams to familiarize themselves with the exam format.
Initial certification is valid for 10 years. To maintain certification, physicians must participate in the Maintenance of Certification (MOC) program, which requires ongoing professional development and knowledge assessment. This assessment can be met through the traditional 10-year MOC exam or the newer Longitudinal Knowledge Assessment (LKA).
