Hypoglycemia can mimic or worsen stroke symptoms and is a time-critical, reversible cause of neurologic deficits. With a measured glucose of 48 mg/dL, the AEMT’s immediate priority is to correct hypoglycemia per protocol—oral/buccal glucose if the patient can protect the airway and follow commands, or IV dextrose/IM glucagon if oral administration is unsafe or IV access is available/indicated. Treating glucose first does not preclude rapid stroke evaluation; notify the receiving facility of a suspected stroke with corrected hypoglycemia and continue transport. Failing to correct hypoglycemia (A) risks ongoing neuronal injury and persistent deficits. Hyperventilation (C) is not indicated and can reduce cerebral perfusion. Aspirin (D) is used in acute coronary syndrome, not for suspected stroke in the field; hemorrhagic stroke cannot be excluded prehospital, and giving aspirin may be harmful. Reassess mental status, cranial nerves, strength, speech, and glucose after treatment, and document times (last known well, intervention, recheck) to support definitive care decisions.
